Adult Day Services — provides care and a variety of activities to older adults age 55 and greater, that typically have limitations of mobility, stamina and/or cognitive functioning.  Programs: Hmong DayElders, North DayElders and Southwest DayElders.
   

Assisted Living Programs — provide participants with around-the-clock supervision by nursing and other professional staff, as well as assistance with “Activities of Daily Living” (ADLs) provided by universal caregivers onsite in our existing rent-subsidized communities.

Care Management and Consultationprovide individuals, families, attorneys, trust departments, and guardians/conservators with comprehensive services to assess and monitor the needs of the older adult.

Caregiver Servicesoffers assistance, information, counseling, and respite services to family members who are caring for an older adult at your home or theirs. 

ElderRide Transportationhelps seniors, age 60 and older, remain independent and active by providing them with transportation to medical appointments, grocery shopping, senior dining, senior center activities, recreation, and other destinations within the Minneapolis city limits.


Experience Corps — engages people over 50 in boosting literacy and academic performance of children in urban schools and after school programs.
 

Highrise Social Services Programstaff work in 40 of the Minneapolis Public Housing Authority apartment buildings to provide case management, information and referral and group services to residents who are age 60 and older and/or are disabled.


Hmong Elder Connectionsoffers positive cultural, educational and social experiences for seniors of the Hmong community to help preserve their physical and emotional health and well-being.

M.A.O. Legal Services — assists clients with wills, trusts, estate planning, probate, powers of attorney, health care directives, guardianship, conservatorship, special needs trusts, Medical Assistance planning, and real estate matters.


Park ElderCenter — a welcoming, comfortable community center that offers a wide range of services to support the health and independence of older adults. 


Protective Servicescan provide telephone consultation, information and assistance, referral, and multi-dimensional assessments to adults having difficulty with financial management or personal decision-making or who are in need of guardian and/or conservator services.

RSVP (East Central Minnesota Mora, MN)identifies volunteer opportunities for its members, who are age 55 and over, that meet their individual skills, interests, schedule and geographic availability. 

RSVP (Twin Cities Metro Area) — identifies volunteer opportunities for its members, who are age 55 and over, that meet their individual skills, interests, schedule and geographic availability. 

Senior Mental Health Servicesoffers age-specific diagnostic and treatment options to address the specialized mental health service needs of older adults.

Senior Nutrition Program offers people age 60 and up nutritional meals and services including congregate dining, home delivered meals, and individual nutrition education and consultation.


Southwest Center — community center offers a wide variety of programs, services and volunteer opportunities that support the health and independence of older adults.
