Children's Mental Health Case Management Program

Location
:  Volunteers of America of Minnesota, 7625 Metro Boulevard, Minneapolis, MN  55439

Days/Hours: Monday–Friday, 8:30 a.m.–4:30 p.m.

Participants are
: Children with severe emotional disturbances who live in Hennepin County

Fees: Funded through Hennepin County or Medical Assistance

The Volunteers of America of Minnesota Children’s Mental Health Case Management program offers coordination of services enabling children with severe emotional disturbances to remain at home.  Services may include mental health, social, educational, health, vocational, recreational, volunteer, advocacy, transportation and legal services.

 

Each participant’s family is assigned a case manager who assists them in determining what services the child needs, helps the family obtain the services, assesses new needs as the child grows, and coordinates with all service providers who work with the family to keep the services working effectively for the family.
