Participants are: Children and Adults with severe emotional disturbances and/or mental health issues who live in Hennepin, Stearns, Benton, Mille Lac, Wright counties

Description
The Volunteers of America of Minnesota Children’s Mental Health Case Management program offers coordination of services enabling children with severe emotional disturbances to remain at home.  Services may include mental health, social, educational, health, vocational, recreational, volunteer, advocacy, transportation and legal services.

Our Adult Mental Health Case Management offers coordination of services for adults with severe mental health issues to assist them in remaining in their community.  Services include mental health, social, health, vocational, recreational, volunteer, advocacy, transportation and legal services.

Each participant’s family is assigned a case manager who assists them in determining what servicesare needed, helps the family obtain the services, assesses new needs s, and coordinates with all service providers who work with the family to keep the services working effectively.
